Hey! A Note to any Gamemaster!!!!!

I'm sitting here and filling out this character transfor form.. But I have a few question I would like answered if possible..
I'm moving a character from an account, to my original account..

1. The form asks for an email adress to that account, that the char is getting transfered to. However i have no email .. Is it possible to leave a blank ? .. And to be honest I cannot see the point  typing the email for the account its getting transfered to.. What can u use that for anyway ? ..

2. at the point Owner of Account To Transfer From's Personal Information
This, along with account password, will be used to verify
that the transfer is approved of. Please fill it out in
the same way you filled it out when you ordered your
Citizenship.
Can I just type in my real name and leave the rest as blanks, if I do not wish to expose my personal information?
- And if not, what kind of information do I type? The same information I used to make my paypal account?
It says please fill it out, same way as you filled it out when you ordered your citizenship.. BUT .. I bought my Citizenship for Gold Piece on Faldon .. so what do i type?

I hope some Gamemasters are able to help me out. Or anyone else who has made a char transfer in the past..
